OBJECTIVE: This randomized crossover trial evaluated periodontal indexes of two types of 3 x 3 retainers (a modified 0.032-in SS V-loop retainer and a conventional 0.0215-in SS coaxial wire retainer) after bonded for six months. Also, bonded failure rate, and a questionnaire about comfort, ease of cleaning and overall preference were recorded. MATERIAL AND METHODS: 15 patients were enrolled in this study who used both retainers for six months each, having a 15-day wash-out interval between each bonded retainer usage. The following periodontal index were recorded: Plaque Index (PI), Calculus Index (CI) and Gingival Index (GI). Patients answered a questionnaire to assess comfort, ease of cleaning and overall retainer-type preference. Rate of bonding failure was also evaluated. RESULTS: V-Loop retainer showed higher PI (P<0.05) as compared to conventional 0.0215-in coaxial wire retainer. However, CI and GI presented no statistically significant differences between both types of retainers. The conventional 0.0215-in coaxial wire retainer was chosen as the most comfortable (p<0.05), although no statistically significant differences were found for all other questionnaire answers. Bonding failure events were more observed in the 3x3 V-Loop retainer (p<0.002), as compared to the conventional 0.0215-in coaxial retainer. CONCLUSION: V-Loop retainer showed higher PI (p<0.05), higher bonding failure rate and less comfortable, as compared to conventional 0.0215-in coaxial wire.

RESUMO Objetivo: Este estudo cruzado e randomizado avaliou os índices periodontais de dois tipos de contenções 3x3 (uma contenção aço V-Loop de 0,032" modificada e uma contenção convencional de fio coaxial aço de 0,0215") após colagem, por seis meses. Além disso, foram registradas a taxa de falha na colagem e um questionário sobre conforto, facilidade de limpeza e preferência geral. Material e Métodos: Foram incluídos nesse estudo 15 pacientes que usaram ambas as contenções por seis meses cada, com intervalo de quinze dias entre cada contenção fixa. Foram registrados os seguintes índices periodontais: Índice de Placa (IP), Índice de Cálculo (IC) e Índice Gengival (IG). Os pacientes responderam a um questionário para avaliar o conforto, a facilidade de limpeza e a preferência geral pelo tipo de contenção. A taxa de falha de colagem também foi avaliada. Resultados: A contenção V-Loop apresentou maior IP (p<0,05) em comparação ao fio coaxial convencional. Entretanto, IC e IG não apresentaram diferenças estatisticamente significativas entre as contenções testadas. A contenção convencional de fio coaxial 0,0215" foi escolhida como a mais confortável (p<0,05), embora não tenham sido encontradas diferenças estatisticamente significativas para todas as outras respostas do questionário. Eventos de falha de colagem foram mais observados na contenção V-Loop 3 x 3 (p<0,002) em comparação com a contenção coaxial convencional de 0,0215". Conclusão: A contenção V-Loop apresentou maior IP (p<0,05), maior taxa de falha de colagem e foi menos confortável em comparação ao fio coaxial convencional 0,0215".

Biofilm on acrylic resin dental prostheses may cause gingival inflammation. This study evaluated the influence of a silicon dioxide coating layer applied onto acrylic resin on the adhesion of microorganisms. Blocks (5 x 5 x 3 mm) of acrylic resin were evaluated for surface roughness and divided into two groups: control (CG) and coated with silicon dioxide (LG group). The specimens were evaluated by scanning electron microscopy (n = 1) and by contact angle analysis (n = 3). For the in situ study, 20 volunteers wore acrylic palatal devices containing three samples from each group (n = 60) for 2 days. The biofilm formed was quantified by metabolic activity and total biomass using the crystal violet assay. The results were subjected to Bartlett's normality test and Gamma model with random effect for the response variable (α = 5%). The mean contact angle of the coated group was significantly lower than that of the uncoated group (p < 0.05). The metabolic activity of microorganisms in the biofilm on the blocks treated with coating was significantly lower than that of control blocks (p = 0.02). Regarding the amount of extracellular matrix produced by the microorganisms, there was no difference between the CG and LG group (p = 0.05). The application of a silicon dioxide coating on acrylic resin reduced the activity of the polymicrobial biofilm formed in situ. This coating may be advantageous for patients with conventional complete dentures or implants made of acrylic resin and who have motor difficulties that prevent them from cleaning their prostheses properly.

Purpose: The purposes of this study were to: (1) compare dental hypersensitivity (DH) between hypomineralized permanent first molars and unaffected controls molars in patients with molar-incisor hypomineralization (MIH); and (2) assess the influence of opacity, color, and subject age on DH. Methods: This cross-sectional study identified 40 children/adolescents, who were attending a children's dental clinic and had one to three first permanent molars hypomineralized (demarcated opacities). The severity and color of the hypomineralized lesions were also evaluated. The assessment of DH was made using evaporative stimulus using a jet of compressed air applied by a dental syringe for one second, perpendicular to the occlusal surface employing the visual analogue scale (VAS) and the Schiff cold air sensitivity scale (SCASS). To verify the association between the presence or absence of DH and MIH, Pearson's chi-square and Fisher's exact test (P<0.05) were performed. Results: The frequency of DH was 28.0 percent in hypomineralized teeth and 9.4 percent in normal controls (by VAS; P=0.002). A higher frequency of opacities was observed (P<0.001). Older individuals had DH more frequently (P<0.05). Conclusions: Hypomineralized molars presented a higher frequency of dental hypersensitivity than control teeth, and dental hypersensitivity was associated with darker opacities of the lesion and older-aged subjects.

OBJECTIVE: The aim of this paper is to present the results of a consensus meeting on the threshold property requirements for the clinical use of conventional glass-ionomer cements (GICs) for restorative indications. METHODS: Twenty-one experts on GICs evaluated the results of tests on mechanical and optical properties of 18 different brands of restorative GICs: Bioglass R [B], Chemfil Rock [CR], Equia Forte [EF], Gold Label 2 [GL2], Gold Label 9 [GL9], Glass Ionomer Cement II [GI], Ionglass [IG], Ion Z [IZ], Ionomaster [IM], Ionofil Plus [IP], Ionostar Plus [IS], Ketac Molar Easymix [KM], Magic Glass [MG], Maxxion R [MA], Riva Self Cure [R], Vidrion R [V], Vitro Fil [VF] and Vitro Molar [VM]. All experiments were carried out by a team of researchers from Brazil and England following strict protocols, under the same laboratory conditions throughout, and maintaining data integrity. RESULTS: There was consensus on: determining as primary properties of the material: compressive strength, microhardness, acid erosion and fluoride release, and as secondary properties: contrast ratio and translucency parameter, in order to rank the materials. Seven brands were below the thresholds for restorative indications: IZ, IM, IG, MA, VF, B and MG. CONCLUSIONS: Based on the primary properties adopted as being essential for restorative indications, the conventional restorative GICs that met the thresholds and could be considered suitable as long-term restorative materials were: EF, GI, GL9, KM, IP, GL2, IS, CR, V, VM and R. A decision-making process to select the best GIC must also include results from clinical trials. CLINICAL SIGNIFICANCE: This study provides a ranking of GICs that could be considered suitable as long-term restorative materials based on their main properties.

OBJECTIVE: To evaluate the translucency parameter (TP) and contrast ratio (CR) of different conventional restorative glass-ionomer cements (GICs). MATERIALS AND METHODS: Eighteen brands of GICs were evaluated. Five disks of each material were made following ISO 9917-1. The luminous reflectance and Central Bureau of the International Commission on Illumination parameters of disks were evaluated using a colorimeter, against backings of white and black, to obtain the translucent parameter and contrast ratio of different brands of glass-ionomer cements. The correlation between translucency parameter and contrast ratio was assessed with the Pearson correlation test. The translucent and contrast ratio parameters values were submitted to the one-way ANOVA and Tukey test for multiple comparisons (p < 0.05). RESULTS: There was a strong inverse relationship between CR and TP (r2  = 0.94, p < 0.001). The contrast ratio decreased as translucency increased. There were significant differences in TP and CR among brands (p < 0.001). CONLUSIONS: GICs exhibit different translucency and contrast ratio behavior. Some brands of GICs presented very low TP and this condition would be unacceptable for areas with esthetic demands. In addition, TP and CR showed a strong linear relationship. CLINICAL SIGNIFICANCE: The results found in this study demonstrated that the knowledge of the translucency and CR of different conventional restorative GICs is important in order to guide clinicians in the selection of restorative GICs for anterior teeth.

Acrylic resin has been used in the manufacture of prostheses, however, in the oral cavity, this material starts to retain microorganisms capable of causing gingival inflammation due its porosities. The aim of this study was to evaluate the influence of the use of silicon dioxide as a coating layer applied onto acrylic resin, on the adhesion of Candida albicans (Ca). After the incubation period in Sabouraud Dextrose Broth, a total of 1 ml of the Ca suspension was added to plate wells, each well containing a specimen of acrylic resin. The adhesion ability of Ca on acrylic resin was determined by counting colonies. Three groups (n = 6) of acrylic resin were assessed: with polishing (RP); without polishing (RW); with polishing and coating layer of silicon dioxide (RPC). Ca deposited on the surface of the acrylic resin was also observed using Scanning Electron Microscopy (SEM). Statistical assessment by Kruskal-Wallis and Student-Newman-Keuls Method were done (α = 2%). There was significant difference among the groups. The RPC group showed the lowest growth, with an average of 5.59 Log CFU/cm 2 ; there was a statistically significant difference in relation to group RW, which presented a growth of 6.07 Log CFU/cm 2 and to group RP with 5.91 Log CFU/cm 2 (p < 000.1). SEM images demonstrated that in the RP and RPC group, the surface of the resin had greater regularity, and smaller number of microorganisms. The application of silicon dioxide coating on acrylic resin appears to be a promising alternative, and its use can help in reducing the adhesion of Ca in prostheses.

The objective was to evaluate the compressive strength (CS), diametral tensile strength (DTS), flexural strength (FS), and Knoop microhardness (KH) of different conventional restorative glass-ionomer cements (GICs) and to correlate these mechanical properties (MP) with the stabilization time (ST) of their chemical bonds. The mechanical strength tests were performed in a universal testing machine. KH readings were done with a diamond indenter. STs were examined by Fourier Transform Infrared spectroscopy (FTIR). Data were analyzed with ANOVA and Tukey test (p<0.05). The Spearman rank test was used to evaluate the dependence between the MPs and ST results. The highest MP values were EF, GL2, GL9, GI and KM and the lowest for MG, MA, B, VF and IM. The longest ST was for GL2 and the shortest was for B. ST correlated positively with MP. GICs with longer chemical bonds ST are generally stronger and the ST value obtained from FTIR was useful in predicting the strength of GICs tested.

PURPOSE: To evaluate the remineralization effects of Bioglass 45S5 (BAG) on dentin composition, adhesive-dentin bond strength, as well as interface and diffusion zone thickness. MATERIALS AND METHODS: Dentin specimens were assigned to a control group (CG), in which the adhesive was applied following the manufacturer's instructions, and a remineralized group (RG), in which remineralization treatment was carried out by rubbing a remineralization solution (0.015 g of BAG with 1.35 ml of distilled water) on the etched dentin surface for 30 s before applying the adhesive. For bioactive analysis (n = 10), control and remineralized dentin were investigated using micro-Raman spectroscopy (mRS) and scanning electron microscopy (SEM). Stick specimens prepared with a three-step etch-and-rinse adhesive were submitted to a microtensile bond strength (µTBS) test (n = 10) after 24 h (24 h) and eight months (8 m). Micro-RS 3D-maps (n = 10) characterized the adhesive-dentin interface composition and diffusion zone thickness, and SEM images (n = 10) evaluated interface thickness. Data were analyzed using Student's t-test or two-way ANOVA and Tukey-Kramer's post-hoc test (α = 0.05). RESULTS: Remineralization treatment increased the mineral content of dentin. Mean µTBSs were statistically different at 24 h, with RG higher than CG; however, this difference was not significant at 8 m. When the adhesive was applied on remineralized dentin, its penetration was reduced, its physical interaction with phosphate was improved, and its degree of conversion increased. The diffusion zone in the CG did not differ from that of the RG, and interface thickness values of the CG did not differ from that of the RG. CONCLUSION: Remineralization treatment promoted mineral growth on the dentin surface, improved the interaction of dentin with adhesive monomers, and consequently resulted in higher immediate bond strengths.

Prevention and health promotion are considered important strategies to control oral diseases. Dental caries is preventable disease and remains the most common chronic disease that affects mainly low income children and still considered the main cause of tooth loss in adulthood in Brazil. The aim of this study is to present a System Dynamics model (SDM) specifically developed with the Stella Architect software to estimate the cost and clinical hours required to control the evolution of dental caries in preschool children in Maringá, Brazil. Two main strategies to control caries were considered in the model: the application of fluoride varnish on teeth presenting white spots, and the use of Atraumatic Restorative Treatment (ART) in cavitated carious lesions without pulp involvement. The parameters used in the model were: number of people covered by a local oral health team = 4,000; number of children up to 5 years = 7% of the population; children's decayed, missing, filled teeth (dmft) index = 2.4; time/cost of 4 applications of fluoride varnish = 5 minutes/US$ 0.716; and time/cost of each ART restoration = 15 minutes/US$ 1.475. The SDM generated an estimated total cost of US$698.00, and a total of 112 clinical hours to treat the population in question. The use of the SDM presented here has the potential to assist decision making by measuring the material and human resources required to prevent and control dental caries at an early age.

The SDM generated an estimated total cost of US$698.00, and a total of 112 clinical hours to treat the population in question. The use of the SDM presented here has the potential to assist decision making by measuring the material and human resources required to prevent and control dental caries at an early age.

AIM: High-viscosity glass-ionomer cements (HV-GIC) are indicated for restorations but their viscosity favors the inclusion of bubbles within it. This study aimed to evaluate the use of ultrasonic and sonic waves in bubble formation, microhardness and fluoride release in a HV-GIC, also considering a different powder:liquid ratio (P/L). METHODS: Twenty-four molars with occlusal cavities were divided into four groups (N = 6): CG, HV-GIC with manual insertion; UG, application of ultrasonic waves; SG, application of sonic waves; and FG, HV-GIC fluid (1:2 P/L) with manual insertion. After 24 hours, bubbles and microhardness (50 g/5 s) were measured. Fluoride release (N = 10) was evaluated in CG, SG and UG after cariogenic challenge for 11 days. Microhardness, total area and number of bubbles, and fluoride release were submitted to ANOVA and Tukey's test. The average size of bubbles was analyzed by Kruskal-Wallis test (α = 5%). RESULTS: FG presented the lowest value of microhardness and higher average size for bubbles (P < .05). Differences concerning total number, total area occupied by bubbles and fluoride release were not found (P > .05). CONCLUSION: The use of sonic and ultrasonic waves had no influence on bubble formation, microhardness and fluoride release of a HV-GIC. Changing the P/L is not recommended.

BACKGROUND: The bariatric surgery may have negative repercussions on oral conditions. AIM: To evaluate the impact of oral health educational/preventive program developed with patients submitted to gastroplasties. METHOD: The sample consisted of 109 patients randomly allocated to two groups: intervention group (IG), where they participated in the oral health promotion program that include multiple educational-preventive approaches; control group (CG), where they received usual care from the bariatric clinic staff, without participation in the program. The oral conditions investigated in the pre-operative and postoperative periods of one month (1M) and six months (6M) were: dental caries, periodontal disease, tooth wear, dental plaque and salivary flow. RESULTS: After bariatric surgery, patients in IG presented: fewer changes in enamel (6M: p=0.004), dentin (6M: p=0.005) and gingival bleeding (6M: p<0.0001), reduction in plaque index (1M, 6M: p<0.0001) and increased salivary flow (6M: p=0.039), when compared with CG. Incipient tooth wear was recorded in both groups (6M: p=0.713). CONCLUSION: There was a positive impact of the implemented program in the prevention of the main oral health problems in patients who underwent gastroplasties, contributing to their quality of life.

OBJECTIVES: To perform a systematic review of test methodologies on conventional restorative glass-ionomer cement (GIC) materials for mechanical and optical properties to compare the results between different GICs. MATERIAL AND METHODS: Screening of titles and abstracts, data extraction, and quality assessments of full-texts were conducted in search for in vitro studies on conventional GICs that follow the relevant specifications of ISO standards regarding the following mechanical and optical properties: compressive strength, flexural strength, color, opacity and radiopacity. SOURCES: The Latin American and Caribbean Health Sciences (LILACS), Brazilian Bibliography of Dentistry (BBO) databases from Latin-American and Caribbean System on Health Sciences Information (BIREME) and PubMed/Medline (US National Library of Medicine - National Institutes of Health) databases were searched regardless of language. Altogether, 1146 in vitro studies were selected. Two reviewers independently selected and assessed the articles according to pre-established inclusion/exclusion criteria. Among all the properties investigated, only one study was classified as being of fair quality that tested compressive strength and was included. It was observed that many authors had not strictly followed ISO recommendations and that, for some properties (diametral tensile strength and microhardness), there are no guidelines provided. CONCLUSIONS: It was not possible to compare the results for the mechanical and optical properties of conventional restorative GICs due to the lack of standardization of studies.

OBJECTIVE: Glass-ionomer cements (GICs) modified with cellulose nanocrystals (CNs) were characterized and evaluated for compressive strength (CS), diametral tensile strength (DTS) and fluoride release (F-). METHODS: Commercially available GICs (Maxxion, Vidrion R, Vitro Molar, Ketac Molar Easy Mix and Fuji Gold Label 9) were reinforced with CNs (0.2% by weight). The microstructure of CNs and of CN-modified GICs were evaluated by transmission electron microscopy (TEM) and by scanning electron microscopy (SEM) while chemical characterization was by Fourier transform infrared spectroscopy (FTIR). Ten specimens each of the unmodified (control) and CN-modified materials (test materials) were prepared for CS and DTS testing. For the fluoride release evaluation, separate specimens (n=10) of each test and control material were made. The results obtained were submitted to the t-test (p<0.05). RESULTS: The CN reinforcement significantly improved the mechanical properties and significantly increased the F- release of all GICs (p<0.05). The GICs with CNs showed a fibrillar aggregate of nanoparticles interspersed in the matrix. The compounds with CNs showed a higher amount of C compared to the controls due to the organic nature of the CNs. It was not possible to identify by FTIR any chemical bond difference in the compounds formed when nanofibers were inserted in the GICs. SIGNIFICANCE: Modification of GICs with CNs appears to produce promising restorative materials.

Resumo Introdução A solubilidade e sorção dos materiais restauradores são considerados fatores críticos, pois podem interferir na qualidade e durabilidade das restaurações. Objetivo Avaliar a solubilidade e sorção de água de alguns materiais restauradores. Material e método Foram confeccionados quatorze espécimes de cada um dos seguintes materiais: Equia® Forte, Z100, Fuji IX/ e Vidrion R, os quais foram levados a uma dessecadora e pesados diariamente até estabilização. Em seguida, foram inseridos em recipientes individuais contendo 40 mL de água destilada cada e divididos em dois grupos de acordo com o tempo de armazenagem: 7 ou 30 dias. Ao final de cada período, os espécimes foram retirados da água, pesados, levados à dessecadora e pesados novamente até estabilização. Os resultados foram analisados estatisticamente utilizando os testes ANOVA 2 critérios e Tukey. Resultado Com exceção do Equia® Forte e Z100, os demais materiais, apresentaram uma variação significativa da solubilidade ao longo do tempo. Quanto à sorção, observa-se que apenas o material Z100 não apresentou diferença significativa em relação ao tempo de armazenagem, mas, na comparação entre os materiais, todos apresentaram diferenças significativas em ambos os períodos. Conclusão Após os períodos de armazenagem de 7 e 30 dias em água, os materiais restauradores Equia® Forte e Z100 não apresentam variação significativa da solubilidade, sendo mais estáveis que os materiais Fuji IX e Vidrion R. Os materiais à base de ionômero de vidro, Equia® Forte, Fuji IX e Vidrion R, sofrem mais sorção em água quando comparados ao material Z100 em ambos os períodos.


Abstract Introduction The solubility and sorption parameters of restorative materials are considered critical factors since they may interfere with the quality and durability of restorations. Objective The aim of this study was to evaluate the solubility and water sorption of some restorative materials. Material and method Fourteen specimens were made (10.0 mm x 2.0 mm) of each of the following material: Equia® Forte, Z100, Fuji IX/ e Vidrion R, then taken to a desiccator and weighed daily until stabilized. They were then placed in individual containers with 40mL of distilled water each and divided into two groups according to the storage time: 7 or 30 days. At the end of each period, the specimens were removed from the water, weighed, taken to the desiccator and weighed again until stabilized. The results were analyzed statistically with two-way ANOVA and Tukey. Result Except for Equia® Forte and Z100, the other materials presented a significant variation of solubility over time. As for sorption, it was noticed that only Z100 presented no significant difference in relation to the storage time, but in comparison of the materials, all had significant differences in both 7 and 30 days periods. Conclusion After storage periods of 7 and 30 days in water, the restorative materials Equia® Forte and Z100 do not present significant variation of solubility, being more stable than the materials Fuji IX and Vidrion R. The glass ionomer-based materials, Equia® Forte, Fuji IX and Vidrion R, suffer more sorption in water when compared to Z100 material in both periods.

RESUMO Racional: A cirurgia bariátrica pode repercutir negativamente nas condições bucais. Objetivo: Avaliar o impacto de um programa educativo/preventivo em saúde bucal desenvolvido em pacientes submetidos à gastroplastia. Método: A amostra foi constituída por 109 pacientes aleatoriamente alocados em dois grupos: grupo de intervenção (GI), onde participaram do programa de promoção de saúde bucal com abordagens educativo-preventivas; grupo controle (GC), onde receberam cuidado da equipe da clínica, sem participação no programa. As condições bucais investigadas nos períodos pré e pós-operatório de um mês (1M) e seis meses (6M) foram: cárie dentária, doença periodontal, desgaste dentário, placa dentária e fluxo salivar. Resultados: Após a gastroplastia, pacientes do GI apresentaram: menor alteração em esmalte (6M: p=0,004), dentina (6M: p=0,005) e sangramento gengival (6M: p<0,0001); redução no índice de placa (1M, 6M: p<0,0001) e aumento do fluxo salivar (6M: p=0,039), quando comparados aos do GC. Desgaste dentário incipiente foi registrado em ambos os grupos (6M: p=0,713). Conclusão: Houve impacto positivo do programa implementado na prevenção dos principais problemas de saúde bucal em pacientes submetidos à gastroplastia, contribuindo para sua qualidade de vida.
